Prerequisite Evaluation Request Form

Prospective students interested in the ABSN, BSN, and/or ELMSN program must submit this request form prior to scheduling a 15-minute appointment with an Admission Counselor.

Please upload unofficial transcripts from ALL colleges attended including any in-progress and/or planned courses at the time of submitting this request.

Upon submission of your request form and unofficial transcripts, our team will return your completed prerequisite evaluation form and provide you with a link to schedule an appointment with an admission counselor.

We will process your request in the order it is received.

International Transcripts: Any international academic transcripts must be evaluated by a member of the National Association of Credential Evaluations Services (NACES) for degree, course content, semester unit equivalencies, and science and cumulative GPAs prior to uploading. International transcripts submitted without the NACES evaluation will not be sent an appointment.
